Location in SystemView: Explore > Beds > Risk of Hospitalisation

What is it?

The Risk of Hospitalisation (RoH) group helps identify patients who are at risk of unplanned readmission or further hospitalisation. It supports proactive discharge planning and care coordination by highlighting those who may benefit from early intervention or follow-up.

Components

1. Readmission Trends
The Readmission Trends component provides daily insights into patient readmissions and trends by risk category. Data can be filtered by specialty, ward, or doctor to monitor patterns over time.
It includes:

  • Summary metrics and trend charts

  • A patient list extract for detailed review

  • Daily updates based on the SystemView RoH predictive model

2. Recently Discharged Patients
The Recently Discharged Patients component lists patients who have recently left hospital and are considered at high risk of readmission. It allows clinicians to quickly identify those who may need early outreach, community support, or follow-up appointments to reduce readmission risk.
